Welcome to MFP-TAC

The Money Follows the Person (MFP) Rebalancing Demonstration enables the elderly and people with disabilities to fully participate in their communities. Enacted by the Deficit Reduction Act of 2005, MFP is part of a comprehensive, coordinated strategy to assist States, in collaboration with stakeholders, to make widespread changes to their long-term care support systems. With the history and strength of the Real Choice Systems Change grants as a foundation, this initiative assists States in their efforts to reduce reliance on institutional care while developing community-based long-term care opportunities.

Who We Are

New Editions Consulting employs a TA team, emphasizing a customized approach toward assisting grantees with successful implementation of their MFP Program. The TA team includes experts in long term services and supports. We work in collaboration with several national organizations and experts to coordinate TA efforts and share information in order to efficiently and effectively support long term services and supports sustainability and the quality of community living for older adults and persons with disabilities.

What We Do

The Technical Assistance (TA) Center for the MFP Rebalancing Demonstration promotes promising practices and provides guidance on program improvement strategies in the areas of system performance and individual consumer outcomes. New Editions Consulting, in partnership with CMS, is available to support the successful implementation of transition and rebalancing efforts to assist people with disabilities and older adults to live independently and with dignity in their communities.
